The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks a Medication Access Coordinator. This position serves as referral point to ensure access to medications for patients by completing prior authorizations, enrolling in medication assistance programs, benefits investigation, coordinating med sync program and implementing referral prescriptions program. They will be responsible in collaborating with staff, patients, pharmacies, pharmaceutical companies, insurance companies, hospitals, and outside providers to identify and resolve barriers preventing patients from obtaining prescribed medications. This position is located at 2130 Stout St., Denver, Colorado 80205 and reports to the Director of Pharmacy.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  1. Collaborate with staff, patients, pharmacies, pharmaceutical companies, hospitals, outside providers and insurance companies to identify and resolve barriers preventing patient from obtaining prescribed medications.
  2. Serve as referral point and expert in submitting applications for patients to receive drugs and copay assistance from pharmaceutical assistance program (PAP)
  3. Coordinate applications and directly enroll patient for assistance meeting drug-specific requirements for participation, including but not limited to forms, patient proof of income, relative clinical information, and provider signature.
  4. Track each request to achieve timely approval & follow-up with appropriate individuals when not approved.
  5. Displays effective file management by maintaining documentation and detailed records of program participation and medication dispensed provided to patients via PAP.
  6. Work together with Enrollment Specialists to enroll patients into Medicaid.
  7. Monitor for and respond to incoming requests for prior authorization.
  8. Performing med sync tasks, including coordinating and aligning prescription refills for patients.
  9. Using EHR and other shared EHR systems to obtain consult notes in compliance with 340B program for referrals, emergency room, urgent care or hospital discharge medications.
  10. Record all services in a timely and complete fashion in the EHR.
  11. Maintain up to date records in EHR
  12. Record telephone and other communications as needed in the EHR
  13. Performs other duties as assigned.

Qualifications Summary

  1. Certified Pharmacy Technician Required
  2. 1-3 years of pharmacy technician experience required
  3. Ability to speak Spanish preferred
