The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks a Residential Services Coordinator to work one-on-one with residents; develop and implement creative programs; establish and oversee partnerships that support residents in successfully maintaining housing; access education and employment services; and resource activities and services that will contribute to a positive sense of community at a supportive housing facility for formerly homeless and very low-income single adults. This position will foster a trauma informed care environment through collaboration with property and programming staff, external partners, and the residents. This position is located at 1425 Xenia St., Denver, Colorado 80220 and reports to the Program Manager.

Essential Job Functions

1. Provide excellent internal and external customer service, behaving in a manner that cultivates trust, respect, and teamwork.

2. Increase participants' access to and engagement in mental health and substance abuse treatment services through an interdisciplinary team approach. This includes working closely with Property Management staff, members of the on-site and off-site care teams, and external partners to increase participants' housing retention, income stability, health and wellness, and social connectedness.

3. Maintain strong communication and cooperation with social services and property management staff. Participate within an interdisciplinary team focused on creating a positive living environment at a supportive housing facility.

4. Establish on-going communication with residents, individually and collectively, to determine areas of need. Work collaboratively with property management, program services departments, community partners, and social service providers to address short- and long-term needs of program participants/property residents.

5. Establish consistent practices for supporting residents in identifying, discussing, and resolving issues related to the individual and community environments. Areas may involve, but are not limited to, cooperative living, self-care practices, financial management, medical and mental health needs, and employment.

6. Effectively assess and diffuse crisis situations that may occur on-site by using de-escalation techniques, calling proper authorities in the event of an emergency, and thoroughly communicating and documenting incidents and outcomes as appropriate.

7. Ensure completion of required paperwork and program database entries, including HMIS, EHR, CMT, and all accounting forms in a timely manner. This includes documenting delivered services and/or resident contacts in accordance with grant requirements and CCH documentation systems by the established deadlines.

8. Maintain timely and accurate records of resident activities that impact housing stability (including lease and program violations), as well as the history of interventions and responses to those activities provided by the resident's care team.

9. Develop and implement creative on-site educational opportunities, social services, and community building events based on residents' needs and interests.

10. Coordinate services with case managers/staff from all CCH departments involved (including Residential Services, Housing First, Property Management, Housing Assistance, etc.), internal and external health care providers, community agencies, and other partners to optimize service delivery to individual residents.

11. Attending and participating fully in expected meetings identified by the management or team, including but not limited to department meetings, program grant meetings, committee commitments, Housing Retention Committee responsibilities, Blended Management, etc. as well as being prepared for such meetings in advance.

12. Identify and establish partnerships with other CCH departments, Property Management, and community-based groups to bring educational, financial, health, and recreational and other services to residents of the property.

13. Remain current on knowledge of best practices in substance abuse treatment, mental health treatment, and supportive housing by effectively utilizing clinical supervision, continuing education opportunities, and attending conferences and symposia related to these subject areas.

14. Communicate frequently with the Supportive Services team and other senior management staff to ensure information flows constantly and in both directions.

15. Performs other duties as assigned by supervisor, property management staff, and clinical supervisors.

Qualifications Summary

  1. Bachelor's degree in Social Work or related field required.
  2. Minimum of two years' experience providing educational and/or social services to adults with severe and persistent mental illness or co-occurring disorders.
  3. Experience in mental health, substance use, permanent supportive housing, and crisis intervention strongly preferred.
  4. Demonstrated competency in working with people from diverse backgrounds and ability levels.
  5. Ability to communicate and cooperate effectively with property management staff, MACT team staff, and representatives from other agencies.
