Job Title: Lead Case Manager

FLSA Status: Full Time - non-exempt
Reports to: Program Supervisor

Schedule: FT, Varies
Supervises: Case Manager

Rate of Pay: $29.35

Closing Date: 2/28/2024

Function: This is a full time, non-exempt, hourly position, 40 hours a week with vacation and sick time as well as medical, dental, and optional insurance coverages.

The Lead Case manager conducts intake and ongoing assessments, prepares case plans while assuming a leadership role and directly oversees programing case management staff and interns. Provides individualized and person-centered case management services with the goal of moving families toward self-sufficiency and stability as quickly as possible. This position is critical in modeling and providing guidance and support to staff in a shelter utilizing the Housing First approach to homelessness assistance. This full time, non-exempt role reports to the Lambuth Family Center (LFC) Program Director.

Duties and Responsibilities:

  1. Lead Case Manager
    1. Facilitate weekly case coordination meetings and other programming meetings with case managers, education & activity coordinator, interns and other relevant staff (in conjunction with the LFC Program Supervisor and Director)
    2. Carry a caseload and create individualized, client centered case management services and case plans.
    3. Conduct one time and ongoing assessments including Health Hope Index, Self Sufficiency Matrix and VI-SPDAT.
    4. Create community connections for referrals and partnerships. Serve as a liaison for residents.
    5. Manage case files, creating, preparing, and maintaining accuracy; provide monitoring and oversight for all case management files, ensuring accuracy and providing guidance when needed. Makes recommendations for improvement of file/data storage and systems.
    6. Assist in the coordination of Continuum of Care for program participants as it relates to community partner agencies and MDHI.
    7. Provide oversight to case management from intake to program exit; provide recommendations and strategies to improve and streamline systems and ensure all components are client centered.
  2. Program Oversight:
    1. Acts as direct supervisor for programming staff including case managers, and interns.
    2. Responsible for approving and submitting time sheets and time-off requests for case management staff.
    3. Provide support, encouragement, and crisis intervention to staff and residents as needed. Model non-violent crisis intervention strategies.
    4. Train and onboard new programming staff in the areas of Housing First, Trauma Informed Care, Crisis Management, Data management systems, etc.
    5. Consults with the LFC Program Director on program related decisions.
    6. Work with LFC staff to prepare grant and contract reports in a timely manner.
    7. Other duties as assigned.

Qualification and Education Requirements:

  • Minimum of 3 years case management experience, preferably with homeless populations. Minimum 1 year experience in a supervisory role. Bachelor's Degree in Social Work or Human Services related field required. Master's in Social Work or similar degree, preferred.
