Summary Provides basic care coordination, identifies and reduces barriers to care, provides referrals to necessary resources. Works primarily with the School Oral Health Program's (SOHP) staff and patients and families who need additional dental care including restorative care. Assures that patients have an established dental home, complete treatment plans, and maintain scheduled care. Supports SOHP hygienist and dentist at Florence Crittenton school-based health center as a Patient Access Specialist (PAS). Collaborates with dental clinic teams and community dental practices. Captures and maintains data required for grant reporting (while applicable). Essential Duties and Responsibilities
1.Responsible for all patients who are referred for restorative care from the SOHP.
2.Schedules patients in dental clinics for restorative care.
3.Follows up on patient treatment plans.
4.Responds to EPIC Inbox messages.
5.Schedules patients with School-Based Health Center (SBHC) RDH and DDS.
6.DPS Restorative Program management and tracking:
a.Schedules patients with the Denver Health dental clinics.
b.Maintains referral list.
c.Tracks treatment plans and outcomes.
d.Sets-up families for My Chart.
7.Other duties as assigned.
Minimum Qualifications
Education and Experience
1.High school diploma or GED certificate, required.
2.One year of community involvement and/or working with clients, required.
Knowledge, Skills and Abilities
1.Bilingual English/Spanish, preferred.

2. Knowledge of basic care coordination and community resources.
3. Ability to establish and maintain rapport and a therapeutic relationship.
4. Communication and motivational interviewing skills.
Computers and Technology
1. Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint), required.
2. Proficiency with Sharepoint, required.
3. Knowledge of Epic scheduling software, required.
4. Ability to perform web searches for clients/patients.
Certificates, Licenses and Registration
1. Basic Life Support (BLS), required.
2. Certificate of completion from the Community College of Denver Community Health Worker program and/or the Patient Navigator Training Collaborative at the University of Colorado, preferred.
