The Clinical Social Worker I is a licensed clinician that conducts psycho-social assessments and interventions with patients/families and serves as a liaison with multi-disciplinary care team and external community agencies to implement interventions.

ESSENTIAL FUNCTIONS
An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.
Develops and implements service/treatment interventions that provide a variety of services to meet patient/family needs.
Monitors progress, cooperation, and acceptance of services.
May serve on department and hospital-wide committees as directed.
Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment.
Develops and maintains relationships with peers within and outside of the department as well as community resources.
Conducts initial needs assessment to gather information on the patient/family's medical, social, and financial history.
Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals.
Facilitates staffing, care conferences, etc to coordinate care between the treatment team.
Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations.
Participate in on call and rotation.

Salary Information
Annual Salary Range (Based on 40 hours worked per week): $63,317.59 to $94,976.38Benefits Information
As a Children's Hospital Colorado team member, you will receive a competitive pay and benefits package designed to take care of your needs that includes base pay, incentives, paid sick leave and a robust wellness program.
As part of our Total Rewards package, Children's Colorado offers an annual employee bonus program that rewards eligible team members based on organizational performance. If organizational goals are met for the year, the bonus is paid out the following April.
